Not sure why they do that, but seems they just pull a hand full of foreskin to one side and chop off the excess. Some are better than others, but few have very smooth cuts. You get used to it.

Usually they have it done around 13 years old and even as late as 17 that I know of. Just when a young boy / transgirl is going through that constant erection phase. Something very cruel and foolish about the whole thing, perhaps it's a catholic thing, to incite guilt for sexual thoughts. hmmm
