I don't get the full picture here.
So did Trump say that anybody who is trans won't get medical care?

There was a federal regulation enacted by the Obama administration that prohibited healthcare providers from discriminating against patients who are transgender. The Trump administration (1) began the legal process to rescind the regulation and (2) ordered Department of Justice lawyers to attack rather than defend the regulation in court. What "2" means, basically, is the federal government switched sides from being the defendant in lawsuits challenging the regulation to being the plaintiff suing to overturn the regulation.

The Administration's position doesn't mean that anyone who's trans won't get medical care; what it means is, if a healthcare worked refuses a transgender person medical care because that patient is transgender, that type of discrimination would be legal under the new regulation. Mostly, this type of discrimination has occurred when pharmacists refuse to fill prescriptions related to hormone replacement therapy. However, as the regulation is written, an EMT or emergency room nurse/doctor could, theoretically, refuse to treat a patient solely because that patient in transgender.
