    Not an answer but a speculation.... female is the default in humans. Hormonal levels combined with hormonal sensitivity dictate development.

    Normally if a fetus is exposed to more testosterone, they develop male genitalia. I.e. their gonads drop and become testes, the area that would be a vaginal opening closes.

    But if there is not enough testosterone this does not happen, and you end up with a hermaphrodite. If the testosterone is there but the fetus is not sensitive enough to the chemicals, they end up developing as a girl (called AIS). Hormone levels and the body's sensitivity towards them are probably the key. Politically a lot of people harp on chromosomes and pretend they are static but this is not scientifically valid. Take someone who needs a bone marrow transplant. If their donor is the opposite sex their blood production will now carry the opposite sex's chromosomes. A guy whose donner is a girl will be producing XX blood and vice versa, but this has no consequences on their body. A preteen who gets a marrow transplant does not develop any differently in puberty, because that's what the hormones do. Chromosomes don't mean much.

    If the same is true for the brain... then it could simply be that some XY-fetuses don't get the right hormonal exposure during development, or their brain is insensitive to the testosterone, and stays as the default female.

    With FtM if this is all true you would see it occurring with hyper sensitivity to testosterone or excessive testosterone levels during fetal brain development, which could be far rarer than testosterone shortages or high tolerances. I have no studies to point you here and I am just speculating so take it or leave it as you will.

    But: There are profound differences in brain structure between males and females, so if it is true that trans people have the brain of the opposite physical sex... this should be something that could be proved scientifically at some point in the future. Unfortunately they do not know enough about brains and the money for research isn't flowing like it used to so who knows if... or when... you will get an answer.

    TV is a whole other ballgame. After the French Revolution western civilization went through a dramatic shift known in sociology as the great male renunciation. Essentially what happened is men abandoned showing off their bodies (i.e. cosmetics & sexualized fashion) in favor of drab utilitarianism. Think of the baroque period where guys wore makeup and elaborate clothing. The founding fathers are never depicted in pants... few decades later men were never wearing anything else, abandoned the wigs, lace, makeup etc. Meanwhile sexualized aesthetics for women never went out of style. From a fashion perspective in western society women's fashion is literally designed to turn men on. That is the whole point.

    So what might be happening with TV is that you have people who are turned on by the garments themselves, a sexual quirk/statistical deviation that manifests itself as say... guys putting on hose to jerk off (this is a hyperbolic stereotype but bare with me). The fact that history is void of such TV fetish stereotypes before the French Revolution is probably ominous as to what TV is and how it has come to be. I had an acquaintance whose best friend was a FtM TS, but he was a MtF TV. Meaning he transitioned to male but would put on lingerie to masturbate. TS is rare, FtM is rarer, TV is rare, someone who is all 3 is probably extremely uncommon. But it might go to show the mechanisms involved here.

    Of the 135,367 likely transgender people who changed their names with the Social Security Administration, 65 percent were transgender men and 35 percent were transgender women. Transgender women, meanwhile, were more likely than transgender men to note the change in their sex with the government. (From 2002 to 2013, the Social Security Administration required proof that genital surgery had been completed before people could change their sex in official records, but it no longer does.)
