You will be asked.to fill in a form, on it is question asking who you've had sex with, you need to tick sex with men, they don't have one for TS as biologically TS fall in the man having sex with men category as that group is at higher risk for STD's. just be honest with them, they not their to judge and have seen and heard it all before. The more honest you are the better the check up.

You should tell because it's better to be open and honest with them in regards to your health as men who have sex with men (TS fall in their) are more at risk then heterosexual people.
