It's a quick clip without a good clear view, but I think there may be a couple of things going on.

The lateral curvature may be due to Peyronie's disease, a disorder of the connective tissue in the penis. The loose and engorged foreskin makes me think she's a penis pumper. A vacuum pump is sometimes used to treat Peyronie's.
