Not necessarily, I believe many escorts, like most people, contract the virus from their lovers...though I would assume that many Johns are positive never the less.
But statistically it probably wouldn't take a lot of Johns to spread HIV...especially amongst a niche business. Look at any review thread and see how many providers a single client sometimes sees.

Actually, it wouldn't shock me if there actually was much less fear about dying from aids given the drug regimens that are available now, which can drive viral loads down so low that HIV+ people who adhere to them won't even test positive.
