I would say that may in Madrid is not as good as in other months. (Its still fun however).

Many girls TS or otherwise go elsewhere during the summer.

I would say that summer is better in Ibiza and other places with beaches.

In virtually every country the only action is in a big city, hence the wording of the question, but in Thailand I have found action even in very small places. Patong on the island of Phuket is only a town but has probably more choice than London. However, when looking at the best cities and using value; a large choice; ease of meeting and safety as criteria then my list is :-

Statistically NYC is number One for highest Transgendered Population... Number two would surprise most - Its Minneapolis, but you wont find the mega TS parties here like LA or NYC sorry to say...
