Well Poison is one of the most popular transgender characters in gaming, and this is her first appearance as a playable character.

Did you know? The character Poison from Capcom's Final Fight and Street Fighter games is a transexual with different operative status in different markets. In Japan she's non-operative, but in America the feeling is that she must be post-operative to be acceptable.

This was confirmed by Street Fighter IV’s producer Yoshinori Ono, who explained why Poison was not included in the new game: "Let’s set the record straight: In North America, Poison is officially a post-op transsexual. But in Japan, she simply tucks her business away to look female." He later emphasized it again when asked about what female characters could be included in the game Street Fighter IV, stating that it would be too confusing to include her due to the region-specific gender.

The reason she's Transsexual though is fucking hilarious. They didn't want a game where you beat up women, so they gave her a dick and the game got the green light.

Poison was originally from Final Fight (a street fighter spinoff, though originally made to be a sequel to the 1st SF game), her appearance pre-dated any female appearing in a fighting game. Street Fighter II (with Chun Li) wasn't released for like 3 years after Final Fight.

She's a bit of a trail blazer.

No its Capcom's creation, they and KOF are very similar, and there have been many jokes in each company's games mocking the other.
