Judging by the skin wrinkling that is an older person so I would reckon a bed-sore (a bad one) or photoshop maybe.
Pumping can cause abscesses and several well-known girls show the scars, including Dany Evangelista, Yasmin Rios and Tayla Diekmann.

I think it looks photoshopped. Cant really tell since its so small but thats my take on it...although it looks pretty gross.

Pretty gross.
I do know that sometimes the silicone can dribble down the legs to the feet though. I've seen that happen to a few girls.
