If you want a street scene the area around the Castello Sforzesco was vibrant some years ago, and it might be possible to nip into the woods of the Parco Sempione depending on how many changes the police might have enforced since I was there. Quality was good at one time, but as with Milan generally little English be the girls Italian or mostly Latin American. There is another street scene in the north on the Viale Certosa certainly in the vicinity of the Raffaello and Mirage hotels.

These references work best when someone who has actually been there gives us a report. If you walk on up the Viale Certosa, it will run into the Corso Sempione which runs from the back of the Sforzesco, you ought to find girls there. There is also a scene on the Piazza Trento, but mixed; as is also the case around the San Siro stadium when there are no matches on, but all at your own risk. 11pm to 2am is the optimum time for the street scene. There are streetwalkers along the main road out of town towards Malpensa airport but you would need a car to take advantage of it.

I can confirm that Nuova Idea in Milan is a good TS club.

Also, I would say the street scene next to Parco Alessandrina Ravizza on viale Toscana (near to the already mentioned Piazza Trento) has an active and consistent scene. Taken lots of drunk taxi rides through the area and many bulges on display with not much left to the imagination...
