The only time I saw a pre-op transsexual in non-sexual material was Eva Robins in a 1982 Dario Argento horror film called "Tenabre", aka "Unsane". Back then, I didn't know she was a TS until just a few years ago.
According to IMDb.com, this is how she became a transsexual:
"She's not exactly a transsexual but suffers from a relatively rare syndrome: in spite of being genetically a male and looking like a normal boy during childhood, after reaching puberty, she naturally developed female secondary sexual characters instead of male."
