The porn industry has a fairly effective and rigid way of regulating safe sex and std's even on bareback shoots.

There has only been one outbreak in the industry, in 2004, which was quickly contained, when one considers the turn over of bb scenes on a yearly basis, one has to consider that these measures are working.

Outside of that industry, I believe unless someone is in a committed long term relationship they should always use condoms - the risk isn't worth it, that I know.
