Law and Order: SVU had two well done episodes. The first dealt with a set of twins ( a boy and a girl) who turned out to both be boys (one's circumscsion(sp) went awry.
The second episode involved a rape murder. The girl who was almost raped turned out to be a TS. When her boyfriend found out he committed suicide. The next part of the episode dealt with her attempt to get transfered to a female prison. She ended up getting assaulted in rikers.

Recently I saw an interesting TV programme on Discovery- the true story of a boy named Bruce Reimer who was conditioned by an 'expert' psychiatrist into being a girl after a botched circumcision left him with little genitals to speak of. The procedure was previously applied to hermaphrodites only. The newborn underwent various procedures and took hormones and lived all his young life as Brenda. Unhappy throughout his/her childhood, young Brenda behaved very much like a little boy. Eventually Brenda's father told his rebellious child the truth. Brenda's first though was to hunt down the doctors who botched his circumsision. Instead, he left home and tried to begin his new life as a man, becoming David. He commited suicide last year aged 38.

I just thought of another tv show which featured TS girls: Sex in the City, when Samantha moved to the meat packing district and had a neighborhood war going on with the trannys living on her block, out loud and late at night. They fought back and forth, until Samantha had the wits to invite them over for a party, and they made friends.
I don't know the NY scene very well, though the girls in the episode seemed to represent that scene pretty well. Most were definitely not passable as I recall.

Don't think I've seen this one so far:
"It's Different For Girls", a movie (caught it on Showtime) about the blossoming relationship between a post-op t-girl and her male friend from her childhood, when she was a boy.

Here's another from this season, sensitively portrayed: Veronica Mars. On one episode, the teenage sleuth helps a friend track down his missing father, only to find out that his father had SRS and is now a woman, living with a man fifty (iirc) miles away. At least once a week, the post-op father drives to the video store the son works at to rent videos and interact a little bit with her son without giving away that she's his father. The kid freaks at first, but later seems to come to terms.

Like a year ago there was a post op on Oprah, if that counts for anything.

Iron Ladies (Satree lek)
One of my favorite tgirl movies which I haven't seen mentioned yet is Iron Ladies. It's been called a sports movie with a difference: the true story of a Thai volleyball team composed of gays, transvestites and transsexuals which won the Thai national title in 1996.
This is a very nicely made movie about transgendered people and it's been one of the most popular international releases from Thailand. There's been a sequel as well (but haven't seen that one yet).
I first saw Iron Ladies at a big outdoor summer screening here in Amsterdam which was part of the yearly gay pride weekend, but it's also on DVD. I can recommend it as an antidote to films about transsexuals with tragic endings and as a good way to practice your Thai

For a exhaustive list of movies (thousands of them) containing at least one spot related to transgenderism see:
