Wild speculation but when you develop if you are a guy or XY then your testicles form way up where the ovaries form. When you reach a certain age usually between 6-9 they should be in the bag. High Orbiting testicles are quite common and that scar looks like one on my scrote, it is from where the surgeons have to tie the tubes to the wall, this can get quite painful during sex. Its kinda like being kneed in the balls when you pull back, so they enter again and extend the tie-back, that is what the central scar looks like to me.
