The technology to grow organs from one's own tissue exists, but very little effort is being put into developing it because there's little profit in it.
Regardless of what an individual preference is (whether it's for a vagina or a simulated vagina) there's no question about telling them apart.

To the guys that think it's surgically impossible to achieve a realistic functional vagina, take a look at these. I don't know which Dr.'s were responsible for the results but I think they're amazing!

Please understand where I'm coming from on this. If someone born male says she's always felt like a female, I accept that she is a female, regardless of genes, hormones or body parts--because I believe that a person is a mind rather than a body. When the dick goes, I'm not thinking about how that's one dick I'll never get to suck or get fucked by, because the odds are a million to one that I never would have gotten together with that person anyway--and my liking for dick is nothing compared to the intensity of feeling it would have to take for someone to want GRS.
Furthermore, I believe it's possible to grow an actual vagina (as well as other organs such as hearts, livers, etc.) with enough development of medical technology in that area. Baby steps have been taken over the past 20 years, but the big money goes into drug research because that's where the profit is--not in making a person well, but in making them comfortable in their illness or keeping them alive by selling them drugs for the rest of their life. People will pay an incredible amount of money to be comfortable, and will pay anything to stay alive.
I advocate putting a hundred times more money into organ generation and gene therapy as is being spent now. Someday it will probably be possible to change ones' genes from male to female or vice-versa. That's what I'm talking about! Medicine! Growing not just a vagina from one's own tissues, but an entire reproductive system, so she can have kids if she wants, and never has to take hormones because her body will naturally produce female hormones in the proper ratio.
Medicine is a big deal for me. As bad as socialist medicine is, capitalist medicine is no better and in some ways worse, because the focus is on profit rather than making people well.
And if you're a man trapped in a woman's body or a woman trapped in a man's body, you aren't really well until your body has been fully changed in the best way possible. GRS in its current state might be the best option available right now but it's very poor compared to what could be done right now if medical research was guided by the desire to heal people rather than to profit from them.
