So how does that play out for TS who are increasingly transitioning at a very young age, sometimes taking hormones well before their 18th birthday? I can only imagine that, not having gone through male puberty, their parts down there should offer very little skin material to begin with.

No, but a) the scrotal sac seems to wither away all on its own after orchiectomy, and b) if you start hormones before or in early puberty, your testicles should also remain small, i.e. your scrotal sac will never have grown to the size of an adult or near-adult male in the first place. So my guess is they will go into SRS with a similarly small amount of genital tissue as somebody who had orchiectomy past-puberty and lets a number of years pass before actual SRS.
From what I've heard, surgeons recommend regular stretching exercises for anybody who chooses to have an orchi done before SRS, to keep both the penis and the scrotum from shrinking too much. Don't know if that's true.
