I know Cali state govt. says pornstars must test (or they did at one stage), but then Cali is just one state and if (say) Alabama doesn't have such laws or Florida, then

I would assume this would between rare and 'not a thing' for several reasons:

TS pornstars know the risks of infection.
They are reminded of the risks every time they are tested.
HIPPA would prevent others from outing them if they become HIV+.
They would not out themselves, as they would need money and both TS and HIV+ often lead to job discrimination in the US.

Anytime a performer does a scene they have a current test which is no more than 30 days old. most of the time the testing is even more frequent.

conversly the fine folks at aids.gov recommend once a year for people ages 15-44.

Doesn't matter because the industry has been steadily moving out of LA to Nevada.

There has been like an 80% drop in porn film permits from 2011.
