Well, I do know of cross dressers who fool themselves into thinking they're transsexuals. I've personally known a few. A lot of times they freak out when they try hormones. I can't speak for porn, but I think that stuff is more likely to happen in escorting or prostitution. They present as a certain way to take advantage of the fetish. It does happen. There was a well-known Brazilian girl, Lisa Lawer I think was her adopted name, went back to being a man and married his cousin.
