I doubt that they are fixed there isnt enough at stake to bother with putting the effort into fixing - its mostly recognition and appreciation, sure maybe a little prize and a few more members to the site but all in all its not like we are talking million dollar Nobel prize the world isnt changing over the results - its porn after all...

That being said with fan driven voting a new girl with a good active social fan base can come out ahead of well established models who have been around longer because there are only so many times the girls can rally their fans into voting for different awards before some of the older fans feel they have done that enough times already. This can lead to some surprises but that in turn can help to introduce new models to a crowd that didnt know about them before and keeps the scene fresh and interesting.

Also popularity can be deceptive because if your fan base is just a fan because of your looks or a movie you did might not show up for a fan voting contest whereas Madison's fans were RT her tweets, her facebook was pages and pages of fans saying they voted and telling their friends to vote ALSO some of her fellow ts friends were doing the same. She really worked it for the entire time via all her social networking.

If Madison didn't rally her fan base then she would had loss. We all know that her competition is much more popular than her. However, most of her competition don't communicate with their fans. Sarina Valentina hurt her fan base because everyone believed she pulled a scam with a fake police call.

Madison earned this plain and simple. She's got a strong fan base and she rallied them amazingly well. About 3 weeks ago I saw her activity on face book and knew there was no way to beat that tidal wave of support (at least not this year LOL). If she didn't win, then I might have considered it fixed.

Besides, had she not won, the night would have lacked for the amazing acceptance speech she gave. A REALLY good representation of the trans performer community.
