When I watch porn I tend to have a sketchbook near at hand to notate shots whose composition I like.

Me falling asleep on the toilet jerking off is not a rare occurrence. I wake up, start spankin' again, nod off, wake up, spank, fall asleep again, feet fall asleep too.

Never while but afterwards. The file manager program I use has a section for Description.
For a good scene I'll just make a note there as Great or Keeper. With that I can always find the ones worth watching again or if I need to do some spring cleaning. I can sort via description to group them and delete the rest.
