Washington (CNN) -- The U.S. State Department announced Wednesday a new policy that no longer requires passport applicants seeking a gender change to have undergone sexual reassignment surgery.

The policy, which goes into effect Thursday, allows a gender change with a certification from an attending physician.
The doctor's certification must confirm only that the passport applicant has undergone treatment for gender transition. Limited-validity passports for applicants who are in the process of gender transition also will be available under the policy.
Previously, individuals had to provide documentation from a surgeon that sex reassignment surgery had been performed on them, according to the National Center for Transgender Equality.
The announcement was made to coincide with Gay, Lesbian, Bisexual, Transgender Pride Month, the State Department said.

Submit a U.S. Passport Application in Person:
Read and understand Steps 1 - 7 before leaving this page.

1. Fill Out Form DS-11: Application For A U.S. Passport
Question: Do I fill out the application the way I want it to look on my passport; (name and gender) or do I use the information on my current birth certificate?
2. Submit Completed Form DS-11 In Person
3. Submit Evidence of U.S. Citizenship
4. Present Identification
5. Submit a Photocopy of the Identification Document(s) Presented
6. Pay the Applicable Fee
7. Provide One Passport Photo

To answer my own question,

Yes, you should complete the DS-11 form as you wish for it to appear on your passport. You also need to prepare the following documents:


Completed DS-11Form (https://pptform.state.gov) do not sign the DS-11!
State Issued Birth Certificate
Photo Identification Card
Photo copies of all documents
Payments for book and or card and shipping - as high as $239!
Passport Photograph (http://travel.state.gov/content/passports/english/passports/photos/photos.html)

After you have gathered your documents you need to schedule an appointment (http://iafdb.travel.state.gov)


In a few weeks (depending on shipping and service cost) all of your documents will arrive by mail, they may not all come at once.

I am quite surprised that you didn't have those limited-validity documents in the US.. Polish documents usually expire after 10 years and you have to pay for getting them replaced with new ones. Of course, you may not do that, but you an actually be fined if you are not able to present your ID card during a routine check...

You may find it interesting, but you do not have to have undergone the SRS to get your data changed on any documents. What you do is:
1.See a shrink who is gonna guide you through the rest of the transition's medical part.
2. Sue your parents (lol), saying they were wrong about your gender. (Having passed the medical part you can actually prove you point)
3.Get your name and gender legally changed.
4. Get all the documents replaced.

Of course, apart from the medical examinations, you pay for everything out yer own sky rocket.

Then again, there are people who will give you medz on first app... at a charge of course;p But do not expect the court to believe in their opinions on you needing a sex change;p

I wonder what it's like in Thailand:)

The point of this exercise is to obtain a drivers license that contains my new name and gender.

Recently the federal government of the Uniteed States has started allowing trans people change their gender status on there passport. Because the United States is a Union, all Triple States must acknowledge federal documents. This also includes the Department of Motor Vehicles, which is where I will take my new passport and other doc's to have my drivers license corrected.
