Actually, it's a vasodialor and shouldn't cause any problems after a heart attack unless you're already taking some form of extended release nitrate such as Isosorbide Mononitrate and your BP can drop too low momentarly. Traditionally, a sister compound called amyl nitrate used to be sold in ampules and used like sublingual nitroglycerine for chest angina. Isobutyl or butyl nitrate have been the street legal versions of amyl nitrate. The FDA doesn't seem to be bothered even though it's sold as a headcleaner.
