Meet Poison.

So the story behind her is this. She comes from the Capcom side-scrolling beat 'em up Final Fight back in the late 80's. In which 3 friends Cody, Hagger and Guy attempt to clean up Metro City by literally fighting crime against the Mad Gear gang. Poison is apart of this gang. So yeah...you have to beat the shit out of her to advance. So when it comes time to bring this game over to America from Japan the ESRB or whoever goes through the game and notices that you have to beat up on poor Poison. So whoever was in charge of rating the games says to the creator of Final Fight. "I'm sorry we can't accept this game. It glorifies violence against women and America does not want to market female abuse to it's children" (As opposed to the tons of other violence in the game). So the creator of Final Fight brainstorms and says this. "Poison is actually a pre-op transsexual!" So then I guess that made it okay. And to this very day Poison remains a transsexual. However I think in Japan she is pre-op and over here she is post-op. *shrugs*

Her character design and name are obviously homages to 80's hair Metal bands.

Also Shiek from Legend of Zelda Ocarnia of Time cross dresses, and you never really know about Syfa Belnades from Castlevania 3, though the book calls "him" a "he".

None of them are considered Transsexual. And Samus has always been a gg, it was just a shock that she was because everyone thought Samus was a guy. Poison is the only character I know that is officially transsexual so noted by her creator.

Doesn't really count but, in the first installment of neverwinter nights if you play as a female character [when not using the ugly witch portrait], in one of the taverns of the [believe it was?] 2nd chapter there is a male sprite who hits on you if you talk to him.

You could insult him, buy him a drink, or propose going & having sex to him. If you chose the later, he'll immediately get worried because of how eager you are, and will say that he won't have sex with a girl who is eager because the last time he did, he was tricked by a tgirl (i am paraphrasing, this isn't a direct copy/paste out of the game).

Chrono Trigger was the shit, played through it many many times over the years. At the moment I'm going even nerdier and further off the game radar and playing Xenogears for the original Playstation. Simply phenominal game that surpassed most any of it's time, it's on par with Final Fantasy 7 as far as I'm conscerned. A must play.
