That looks like surgical tape and possibly a bit of spray glue, and no... it's not painful if it's done properly, looks like the above picture would be since she did not pop her scrotum up prior to taping, which would make it painful to sit down. Unless ofcorse she had them removed and only has the sack.
Yeah but if you aren't an export it can get painful.

Another way to do it is a " gaff " which can be made out of a piece of denim and old panty hose ( old school ) or can be machine made. It is basically a thong or T back that's a bit thicker and made out of a stiff but slightly flexible material, such as 3 layers of dance wear fabric.
