Actually, there are a great many theories, and I suspect there's some truth to at least several of them. Like most such traditions, it's rooted in religious beliefs. One thing to note is that 13 is not universally unlucky; the Chinese consider it quite the opposite, a lucky number, and the Egyptians considered it to represent the crossing over into the afterlife (12 representing the number of hours or "houses" in both night and day).

1) Most anciently, it may derive from there being 12 constellations in the zodiac and twelve solar months in the year, but 13 lunar months (actually, there is a 13th contellation in the sideral zodiac, and the 13 lunar months also makes a feminine connection).

2) Most prominently in the West is the fact that there were 13 persent at the Last Supper: Jesus and the twelve apostles, one of whom (the "13th" man) betrayed Christ. This may well be the most basic root to the fear in European/Western culture.

3) Also European, and an older religious tradtion, there was a great banquet given by Odin for the twelve great Asgardian gods. Loki, the mischieveous god, was not invited, but crashed the party, then instigated a challenge to Hod, the blind god of winter, to attack Balder the Good, who was a favorite of the gods. Hod took a spear of mistletoe offered by Loki and obediently hurled it at Balder, killing him instantly. (Previously, at Balder's birth, his mother had solicited a promise from all things to never injure Balder, making him, like Achilles, invulnurable. But like Achilles, he had a weakness: she bypassed the mistletoe, which was so lowly and unassuming that there was no way it could injure Balder.) Loki's betrayal was one of the first signs of the end times, Ragnarok, the destruction of the gods.

4) Going back much further in antiquity, some sources speculate that the number 13 may have been rendered evil by patriarchal religions in the because it represented femininity, as mentioned by the 13 lunar months above. Thirteen had been revered in prehistoric goddess-worshiping cultures, we are told, because it corresponded to the number of lunar (menstrual) cycles in a year (13 x 28 = 364 days). The "Earth Mother of Laussel," for example — a 27,000-year-old carving found near the Lascaux caves in France often cited as an icon of matriarchal spirituality — depicts a female figure holding a cresent-shaped horn bearing 13 notches. As the solar calendar triumphed over the lunar with the rise of male-dominated civilization, it is surmised, so did the number 12 over the number 13, thereafter considered anathema.
