You're right that a certain amount of preparation is required, though the amount required is pretty minimal. The whole douching is necessary view isn't accurate. Taking a shower, either immediately prior or a few hours before, is totally sufficient, so long as someone hasn't had a giant, heavy meal in the last couple of hours.
