Dr. Paul McHugh has been beating this horse since at least 1979 when he closed the gender identity clinic at Johns Hopkins. He likens SRS to “performing liposuction on an anorexic child.” Dr. McHugh claims there is no evidence that SRS has ever improved any patient’s emotional, familial or social relationships for the better. He’s published books, and has been a featured speaker on this topic. His longstanding public position on this issue has hardened in spite of the decades of progress and thousands of trans-women who have had SRS and now lead happy, well-adjusted lives.

I would agree that SRS is not for everyone who might initially consider it. I believe it looms large in the consciousness of every transgender woman in the initial stages of her transition as a possible event in her future. Eliminating the option of gender-reassignment surgeries would be a definite step backward, medically and socially. But then we’re living in backwards times. Here in the U.S. we’re taking a quick and slippery backslide to ‘greatness.’

Here's a more nuanced look at the issues surrounding SRS surgery at Johns Hopkins University Hospital.

I think the key issue is adequate preoperative psychological screening.
